Can anyone help?
I downloaded a sample logbook entry for an annual inspection. Short and
complete but can't find it now or the original source.
Anyone have a copy?

No, but I can give you the entry I make when I do an annual:
"This aircraft has been inspected in accordance with the requirements for an
annual inspection, and has been found to be in an airworthy condition.
(signature)
(printed name)
(certificate number)"
This, or some variant thereof, is what I've seen over, and over. Remember,
they have a much harder time hanging you for words you don't put in a logbook.
It's the words you put it that can get you. Short and sweet is usually good.

Are you talking certified or experimental? We do not do "annuals" on experimental airplanes, we do "condition inspections." If experimental, your operating limitations will give you the exact verbiage to use. Mine tells me to write "I certify that this aircraft has been inspected on (insert date) in accordance with the scope and detail of appendix "D" to FAR Part 43 and found to be in condition for safe operation."
Hope this helps,

Technically, you are correct. Change the word "annual" to "condition" in my
blurb, or use the words you propose. If the FAA has approved documentation for
any aircraft, it normally supersedes all other documentation.
The appendix you cite is also the one normally used when doing an annual on an
aircraft for which no type certificate exists.
