Here is an example of how this form was used; it should give you an idea of how the form operates.
It represents a fairly typical flight I would do between Samoan islands in the AC-680E.

Attached is a W&B spreadsheet for my current plane.
You can modify it for any plane by just changing the titles, arm and weights or adding in items as needed. The principle is the same for all aircraft.
