Pulling Cherry Max Rivets

I am using 3/32” Cherry Max countersunk rivets to attach small anchor plates to the back of my skins for attaching access cover plates. Occasionally a rivet stem will break off above the factory head of the rivet. I’m not sure but I think the stem may be pulling further into the shop head of the rivet than needed. Is there any way to prevent this? Do I need to use a small washer on the shop head side?

For some reason the cherry max flush rivets are prone to this. I have
hundreds of them on my Sonex and at least 20 ended up this way. Most of
them can be fix by simply tapping in the protruding piece with a hammer
and nailset.
