I have an opportunity to get a Narrow deck IO540 engine and wonder if anyone knows if it will work with the Van's engine mount and/or if there are any issues with the cowl or anything else.

I had no problems with a B4B5. Everything aligned and fit.Mine was a carb version but I used an airflow performance FI-good luck-Jim 728DD

Depends on price, your desires, etc. Who knows how much longer Lycoming
will support the narrow deck engines. If you want roller cam,
forgedabowdit. If it is fresh overhaul and you don't expect to have more
than 12-15 years of flying left, it would be fine. Some accessories fit
easier on the narrow deck. It has been about 40 years since Lycoming
switched to the wide deck configuration.

I also have a narrow deck + injection and fit was not a problem. I
understood it has been ONLY 30 years since produced but I will not
squabble with a decade. I was also told (take this for what I paid -
nada) that the aerobatic guys prefer the narrow deck units and they are
in relatively high demand. Mine was built up by BPE and the have a
history of doing right by people.

There probably was 10 years between wide deck introduction and stopping production of new narrow deck, so probable both numbers are in the ball park. Your engine has as good a chance as any to make TBO.
