I had a similar issue with mine - I have four of them in my plane (flap, rudder, elevator, and aileron). I ended up replacing three of them through the course of my build. I have not had any issues lately.
I don't know if they caught an electrical spike or a manufacturing defect - but between replacing a bunch of them, putting spike catcher diodes on a couple of my relays, replacing my battery, and ceasing the use of an adjustable power supply to run my electronics in the hangar my issues have gone.
