Can anyone help?
I downloaded a sample logbook entry for an annual inspection. Short and
complete but can't find it now or the original source.
Anyone have a copy?

I assume you mean the annual condition inspection logbook entry for an Experimental Amateur Built aircraft:
“I certify that this aircraft has been inspected on [date] in accordance with the scope and detail of appendix D to part 43, and was found to be in a condition for safe operation.”
The entry will include the aircraft’s total time-in-service, and the name, signature, certificate number, and type of certificate held by the person performing the inspection.
The above entry is required by the Operating Limitations, you should find it stated there.

Thanks for the replies on the condition report (annual) log book entry. I
wholeheartedly agree that the fewer words used, the better the entry.
Now if I can only get my wife to do the same.
