I took a look at the schematics on the CDX-C90 today and found a little information on the pinout for the XA-D210/D211. I've included a snippet of the schematic below that details the specific information related to the XA-D210/D211.
Looks like Pin1=VCC (+5v), Pin2=Data, and Pin3=Grd. This probably means, that for the XA-D210, the TIP is +5v, the next in ring is DATA, and the shoulder is Ground (connection closest to the plastic).
You should verify this with a Volt meter and optionally a oscilloscope.
As far as a digital to toslink converter, you might also want to take a look at your local Radio Shack. They used to have a number of devices for converting. Seems like they were around $20.

I contacted MCS a while ago, they don't ship outside the UK. I have no idea about the Danish company. My girlfriend lives in Germany, so I will probably go that route if I can't figure something out.
The reason I chose the Audio Authority one was because it was the only one that runs off 12v DC. Everything else seems to be 6V-9V. I figured it would be fairly easy to wire it to turn on with the head unit instead of finding a cigarette lighter power adapter or something of that sort.

6-9v is actually what you want. You've already got 5v coming from the C90 for powering the XA-D211. That should be sufficient to power the other devices. That way you don't have to mess with extra power and how to turn it on and off.

Well, this just became a moot point. I found an XA-D210 on ebay. So, I am all set.
But, because the discussion was interesting, I will continue my line of thought. I had thought of using the 5V out of the plug, but that seemed a little more complicated to get at. 12V tends to be very easy to find in the car. I could have ran a wire from the 12V turn on wire or power antenna lead. I think that would have been easier than trying to concoct a way to get the 5V off that plug.
